Telehealth & Community Paramedicine
In partnership with the St. George CDC, St. George EMS, and Jackson Memorial Library — expanding access to care close to home.
Recognizing that health is a foundation for learning, career success, and economic security, we help residents connect to care through an integrated telehealth and community paramedicine program.
Residents can reserve a private space and a ready-to-use iPad with high-speed internet to meet remotely with their own medical providers. Access points include the GRACE Innovation Center, Jackson Memorial Library, the St. George CDC, and the Town Office.
Telehealth access points
These are community access points — private space and technology to connect with licensed providers. They are not medical-treatment sites and do not store medical records.
Operating since 2012, St. George EMS can — on a provider’s referral — deliver goal-oriented care that helps prevent unnecessary 911 calls and hospital visits:
Post-surgical and post-discharge follow-up
Wound care and blood draws
Medication compliance & reconciliation
Support for chronic conditions (hypertension, diabetes, COPD, asthma)
Community paramedicine
